Grant Application Process

All grant applications must be submitted through the on-line grant application system.
After reviewing the grant guidelines, if you have any questions about the appropriateness of your grant request, contact our office at 800-427-1902, extension 5724, or hospicefund@hcr-manorcare.com to discuss your potential grant application.
Please be sure to complete the entire grant application prior to submitting it for review. This includes a completion of the Income and Expense Worksheet or the Community Event Worksheet and any supporting documentation. An incomplete grant application will be returned to the applicant and will delay the decision-making process.
Please provide as much detail as possible on the grant application. It is helpful to provide detailed background and an explanation of need for the request.
Please be sure to list the agencies/resources that have been investigated as other sources of funding prior to applying for a grant. Please include the dollar amount approved from other sources of funding.
Please provide any necessary backup (e.g. statements, invoices, current bills, advertisement of event, etc.) to support the grant application. This information should be uploaded to the field entitled “Supporting Documentation” in the on-line grant application.
If the grant request is for assistance for a family or a patient, complete the Income and Expense worksheet in its entirety. Income and expenses should be provided for the patient’s household. Or, if the patient is deceased, income and expenses should be provided by the surviving spouse and/or legal dependents.
If the grant request is for a community event, complete the Community Event Worksheet in its entirety. Please refer to the detailed guidelines for community events in this manual.
Notification of the status of a grant application will be received within 7 business days. Grant requests for over $1,000 will be reviewed by the Board of Directors and may take an additional 2-3 days for review by the Board.
