The Heartland Hospice Memorial Fund meets its mission by supporting the following types of grants:• Financial assistance for terminally ill individuals and their families who are experiencing financial distress and unable to be self-sufficient due to the circumstances of their terminal diagnosis.• Grief camps• Memorial Services• Education events that communicate the importance of hospice care• Bereavement activities • Funeral and burial assistance• Caregiver transition • Wish fulfillmentGrants from the Heartland Hospice Memorial Fund are coordinated through hospice agencies. The Heartland Hospice Memorial Fund does not make grants for expenses that fall under the responsibility of a hospice agency to provide the appropriate level of care required by the patient (and covered by the hospice benefit).
