Applying for a Grant
All grant requests are coordinated through Hospice locations and grant applications are submitted online using the appropiate application link below:
Heartland Hospice Agency Individual/Family Grant Request

Grant applications must be complete and detailed. If supporting documents are appropriate, they should be uploaded to the online grant application. An incomplete application will be returned to the applicant and will delay the review process.
Notification of the status of the grant application will be sent to the Heartland Hospice Administrator via email and should be received within 7 business days. Grant requests exceeding $1,000 must be reviewed by the Board of Directors and may take an additional 2-3 days for notification.
Emergency grant applications will be reviewed on an as-needed basis. All emergency requests will be responded to in a 24-hour period during regular business hours following the submission of a grant application. All emergency requests should be submitted online; however, a follow-up telephone call to our office will ensure prompt review. Please note, emergency grants are defined as those grants related to meeting the needs of a patient who is actively dying and the patient’s quality of life is being affected.
